As Avrupa İnci Oral and Dental Health Polyclinic (“Polyclinic”), we attach importance to the security of your personal data. Pursuant to the Law on Protection of Personal Data No. 6698 (“Law”) and the Communiqué on the Procedures and Principles to be Followed in Fulfilling the Obligation of Disclosure (“Communiqué”), “Karayolları, Abdi İpekçi Cd. No: 32/D Gaziosmanpaşa/İstanbul”, we would like to explain to our business partners how their personal data is collected and processed as the “Data Controller” defined in the Law. 

Within the framework of this relationship, if there is personal data belonging to another 3rd party other than you in the information and documents obtained within the framework of this relationship, you have informed the relevant data owners that you will share this personal data with our Clinic within the meaning of Article 10 of the Law and for sharing within the meaning of Article 5 of the Law. It is accepted by our Clinic that you have provided the necessary legal reason and it is your responsibility to inform and obtain explicit consent to be carried out in this sense.

At our polyclinic, the following procedures regarding your personal data belonging to you, your employees and your authorized representatives and authorities are carried out by fulfilling the requirements of the legislation.

EXECUTION OF PURCHASING PROCESSES

The personal data processed within the scope of the execution of the purchasing processes are as follows;

• Name and surname, 

• E-mail address,

• IBAN Information,

• Tax number,

• Signature.

Personal data used within the scope of the execution of purchasing processes are collected by the following methods:

​

• Personal data provided by you in physical or digital media are collected by non-automatic methods.

• Collected personal data is stored in Clinic records, physical environment and e-mail environment or on Clinic computers, servers and their backups.

Personal data collected within the scope of the execution of purchasing processes are processed for the following purposes;

​

• Execution of goods / services purchasing processes,

• Execution of business activities.

​

Personal data collected within the scope of the execution of purchasing processes, personal data collected are processed based on the following legal reasons;

• Establishment and performance of the contract.

​

Personal data collected within the scope of the execution of purchasing processes may be shared with the following persons for the purposes described above;

• Natural persons or private law legal entities (Partners, employees, persons from whom accounting services are received).

CONDUCTING BUSINESS AND COMMUNICATION WITH SUPPLIERS

The personal data processed within the scope of the business and communication process with the suppliers are as follows;

• Name and surname,

• Phone number.

 

Your personal data used in the process of conducting business and communication with the suppliers are collected by the following methods;

• Personal data provided by you in physical or digital media are collected by automatic and non-automatic methods.

• Collected personal data is kept in physical environment, e-mail environment, Clinic computers, servers and backups.

 

Your personal data collected within the scope of the business and communication process with the suppliers are processed for the following purposes;

• Execution of goods / services purchasing processes, 

• Execution of business activities.

 

Personal data collected within the scope of the process of conducting business and communication with suppliers are processed based on the following legal reasons;

EXECUTION OF INVOICE PROCESSES  

​

The personal data processed within the scope of the execution of the invoice processes are as follows;

​

• Name, surname, TR ID number

• Information on the invoice on the e-archive and e-invoice invoice

• Phone number, e-mail address.

 

Your personal data used within the scope of the execution of the invoice processes are collected by the following methods;  

​

• Personal data provided by you in physical or digital media are collected by automatic and non-automatic methods.

• Collected personal data is kept in physical environment, e-mail environment, Clinic computers, servers and backups.

 

Your personal data collected within the scope of the execution of the invoice processes are processed for the following purposes;

​

• Execution of finance and accounting works.

 

Your personal data collected within the scope of the execution of the invoice processes are processed based on the following legal reasons;

​

• Performance of the contract.

​

Personal data collected within the scope of the execution of invoice processes may be shared with the following persons for the purposes described above;  

​

• Natural persons or legal entities of private law (persons from whom accounting services are received).

PAYMENT TRANSACTIONS

The personal data processed within the scope of the payment transaction process are as follows;

​

• Name, surname, signature,

• Bank account information, IBAN number.

 

Your personal data used within the scope of the process of payment transactions are collected by the following methods;  

​

• Personal data obtained by you and your personal file in physical or digital media are collected by automatic and non-automatic methods. 

• Collected personal data is kept in physical environment, e-mail environment, Clinic computers, servers and backups.

 

Your personal data collected within the scope of the payment process is processed for the following purposes;

​

• Execution of finance and accounting works.

 

Your personal data collected within the scope of the payment transaction process is processed based on the following legal reasons.

​

• Establishment and performance of the contract.

 

Your personal data collected within the scope of the payment transaction process may be shared with the following persons for the purposes described above;

​

• Natural persons or legal entities of private law (persons from whom accounting services are received).

 

As a data owner in accordance with Article 11 of the Law, regarding your rights listed below,  You have the right to apply to our Clinic as a Data Controller:

​

• Learning whether your personal data is processed or not, 

• If your personal data has been processed, requesting information about it, 

• Learning the purpose of processing your personal data and whether they are used in accordance with the purpose, 

• Knowing the third parties to whom your personal data is transferred, in the country or abroad,

• Requesting correction of your personal data in case of incomplete or incorrect processing and requesting notification of the transaction made within this scope to the third parties to whom the personal data has been transferred, 

• Requesting the deletion or destruction of your personal data in the event that the reasons requiring its processing cease to exist despite the fact that it has been processed in accordance with the provisions of the law and other relevant laws, and requesting that the transaction carried out within this scope be notified to the third parties to whom your personal data has been transferred, 

• Objecting to the emergence of a result against you by analyzing your processed data exclusively through automated systems, 

• Requesting the compensation of the damage in case you suffer damage due to unlawful processing of your personal data.​

Your applications regarding your rights listed above must be submitted in accordance with the "Communiqué on the Procedures and Principles of Application to the Data Controller", in writing and with wet signature, to our clinic address "Karayolları, Abdi İpekçi Cd. No: 32/ Gaziosmanpaşa, Istanbul” to the Avrupa İnci Oral and Dental Health Polyclinic Management by hand, send it via a notary public or to the e-mail address Avrupaincipr@gmail.com,  You can send it via your e-mail address.
